The following revolutionary techniques are included in the Vertical Facelift: the well-known behind-the-ear scar is altogether avoided by making the behind-the-ear cut inside the back of the ear short and extended into the back of the hairline to minimize the scar visualized post-operatively.

What Makes Dr. Chugay’s Facelift Vertical? and Why?

  • A temporal incision is made in the horizontal plane, hugging the hairline. The vector of pull is primarily superior (up), rather than lateral (sideways). This creates the premise for the vertical facelift and leads to a more aesthetically pleasing result. Have you seen that well-known artificial stretched look on patients who have had a facelift? This is due to lateral pull. Unfortunately, most surgeons still prefer lateral pull, as it is easier to accomplish.
  • The lining of facial muscles is sutured upward instead of posterior thus reducing the laxity of facial muscle.

  • Hairline unaffected. Most surgeons make incisions in the hairline to avoid visible scars. However hairline cuts often cause hair loss, which, in turn can make the scars even more visible. What is the best age to have a facelift?

Not everyone ages at the same rate. A great deal of the aging process relates to your tissues and what you do to care for them.

There is no magical age to make the decision to have a facelift. The appropriate time to have a consultation is when you no longer like what the mirror tells you. This may be in your 30s or your 80s. However, it is best to start at a relatively young age, before deep lines and wrinkles set in. It is then a matter of maintenance.

How Long do Facelift Results Last?

There is no magic figure; however, a good estimate is 7-20 years. It is not uncommon for people to have several facelifts to maintain the young look. With Dr. Chugay’s Vertical Facelift technique, your results will last longer than with a standard procedure.

What is a Weekend Facelift? Can I go to Work the Next Day?

A weekend facelift is a term coined by the media. There is no such thing. There is, of course, microdermabrasion. However, it will NEVER give you the same results as a Vertical Facelift.

Dr. Chugay’s Vertical Facelift technique is very aggressive, yet recovery is rapid. A facelift, nonetheless, is considered to be a major operative procedure. Like any extensive surgical procedure, operative incisions and complicated surgical stitching is required. Contrary to popular belief and despite some newspaper and magazine articles, this is not as simple as having your hair set or a getting a facial.

How Soon will I See the Results?

The next day: you will see your new younger look. However, your younger look will not appear beautiful until swelling and bruising subsides. Please, see below for more information about healing.

After Surgery: Healing and Recovery

Immediately after the Vertical Facelift surgery, bandages are placed about the head and face for 24 hours. A scarf may be worn to cover stitches until they are removed. Keep bandages and incisions dry until sutures are removed.

In the Vertical Facelift, patients can expect some swelling of the face and eyelids and some discoloration is common with any type of facial surgery. This is most notable on the second and third days and diminishes in approximately one week. However, this also depends on your type of skin, age and healing ability.

Make-up may be applied over the skin of the face up to the edge of the surgical scars approximately seven to 14 days following the facelift surgery. No hair coloring or stripping should be performed until Dr. Chugay gives you permission. Other hair treatment procedures may be carried out approximately two weeks following surgery.

Do not allow anyone to pull on your hair or roughly massage the scalp. A hair dryer, if used, should be set at medium to cool heat.

Do not wear earrings until sensation has returned to your earlobes.

After your Vertical Facelift surgery, social activities may be temporarily limited. Strenuous exercise should be avoided for four to six weeks.

Eyelid Surgery, Rego Park, New York

Eyelid surgery (Blepharoplasty) corrects drooping upper eyelid skin, tired puffy eyelids, and puffy bags below your eyes – features that may even interfere with your vision, and make you look old and worn out. Many people seek this type of eyelid surgery as early as in their 20’s, 30’s and 40’s to give them a brighter, more youthful appearance.

Eyelid surgery attacks the excess skin and the subcutaneous fat deposits – the sources of the tired haggard look. Blepharoplasty can be done alone, or in conjunction with other facial surgery procedures such as a Vertical Facelift procedure, Forehead Lift procedure or Dr. Chugay’s Crow’s Feet Removal Procedure to remove those stubborn wrinkles around your eyes.

What Elements of Dr. Chugay’s Blepharoplasty Technique are Unique?

In an attempt to somehow deal with the unsightly Crow’s Feet (wrinkles in the outer corner of the eyes as the result of aging), most surgeons extend the incision in the lower lid beyond the outer edge of the eye, which can produce noticeable scarring. Dr. Chugay stops the incision at the outer edge of the eye. For those patients who need to reduce wrinkles around the eyes, Dr. Chugay utilizes his unique technique of Crow’s Feet Removal. The ugly scars are thus prevented, and the wrinkles are dealt with.

The traditional blepharoplasty technique emphasizes extensive removal of fat in the lower lid in order to assure complete removal of the puffy bags. However, this overcorrection often results in hollow or even cadaverous-looking eyes. Dr. Chugay removes fat conservatively and only from certain layers, thus producing excellent results without the hollow-eye look.

Dr. Chugay insists that skin excision on average should be minimal especially on older patients whose muscles are weak and a round-eye appearance can be produced (rather than the normal elliptical shape.)

Dr. Chugay removes some fat in the middle and medial (close to the nose) areas to get a crisper, less puffy upper lid appearance. Presently only about 15% of all surgeons utilize this technique.

To produce a complete rejuvenation in the eye area, Dr. Chugay recommends Crow’s Feet Removal in conjunction with Blepharoplasty. A well-administered chemical peel can later remove the remaining fine lines and wrinkles.
The Surgery

In an eyelid surgery, an incision is usually made in the natural skin fold on the upper eyelid. The excess skin is removed and the skin is repaired with very fine absorbing sutures. The lower eyelid incision is made directly under the eyelash line. The excess skin and some muscle are removed from the region of the lower lids. If there is some fat in the region of the lower lids, that fat is also removed. The skin is closed with fine dissolving sutures.

Risks Involved

Eyelid surgery remains one of the most popular cosmetic surgery procedures. Very rare cases of scarring may occur because the skin is quite thin in the eyelid region. Infection may happen, and antibiotics are given prior to surgery. Bleeding may occur. A condition called Ectropion (pulling of the skin) may happen in the region of the lower lids if an excessive amount of skin is removed. However, the risks are infrequent, and can often be avoided if you follow Dr. Chugay’s instructions carefully.

After your Surgery

Following the eyelid surgery, the eyes may be swollen and tender. Antibiotics will be prescribed for a week’s time. Stitches are removed within several days postoperatively if necessary. Most patients within a week’s time return back to their normal activity. Please, follow all post-operative instructions for optimal and fastest healing and recovery.

Forehead Lift Surgery, Rego Park, New York

A forehead lift or “brow lift” is a procedure that restores a more youthful, refreshed look to the area above the eyes. The procedure corrects drooping brows and improves the horizontal lines and furrows that can make a person appear angry, sad or tired. Dr. Chugay utilizes two distinct techniques of forehead lifting: Coronal and Endoscopic.

Coronal Lift

In the Coronal Forehead Lift, Dr. Chugay begins as he makes an incision starting at or near the ear and running across the top of your head slightly behind your hairline. The forehead skin is lifted up and excess skin is removed, sometimes with part or all of the muscles that cause forehead tension and wrinkling. Dr. Chugay uses blunt dissection to free up the tissues. While using sharp cutting instruments to free up the forehead skin is faster, blunt dissection is much safer for the patient and will result in less bleeding, less risk of nerve and tissue damage, lower risk of hematoma, and faster healing and recovery.

Dr. Chugay uses subcuticular (just below the level of the skin) sutures for the final closure of the incision after the surgery. This technique is time consuming and difficult to execute; however, it leaves a more natural and aesthetically pleasing final result. A vast majority of surgeons continue using removable sutures, which leave hash marks and increase hair loss.

Both blunt dissection and subcuticular suturing, along with other aspects of Dr. Chugay’s technique will reduce your chances of hair loss and unsightly scarring at the area of incision.

Endoscopic Lift

Instead of a large incision in the hairline area as in Coronal Forehead Lift, several “puncture-type” incisions are made just at the hairline. Using a special tool called an endoscope (a special viewing instrument that allows a surgeon to see images of the body’s internal structures), Dr. Chugay will remove the muscles that produce frown lines and will reposition the eyebrows at a higher level.

Endoscopic surgery is a minimally invasive approach to surgery and has been used for decades in a number of different procedures, including gallbladder removal, tubal ligation, and knee surgery. However, endoscopy is a very recent phenomenon in aesthetic surgery. Despite its clear effectiveness, endoscopic technique is used by only about 10% of cosmetic and plastic surgeons.

Even though this is a relatively new procedure, Endoscopic Forehead Lift has acquired a standard approach and technique. The Standard Endoscopic Lift technique involves three horizontal cuts through which endoscopic and cutting instruments are inserted to perform the procedure.

Nose Sculpting Surgery, Rego Park, New York

Dr. Chugay uses soft Gore-Tex nasal implants in nose sculpting surgery for certain types of secondary cosmetic and reconstructive rhinoplasty. These instances include augmentation of the nasal bridge or filling in concave deformities, often found in asymmetrical, injured or misshapen noses. These nasal implants (nose sculpting) produce a very natural appearance of the nose, smooth contours, which usually cannot be achieved (regardless of the surgeon’s skill and good will) with cartilage grafts or other natural or synthetic implants or fillers.

For some instances of reconstructive rhinoplasty, Dr. Chugay uses cartilage grafts. Dr. Chugay believes that use of cartilage grafts should be limited to only certain instances in reconstructive rhinoplasty. Cartilage grafts are pieces of patient’s natural cartilage material (removed from nose or ear), which are placed in the patient’s nose. Once the cartilage graft is fully healed and assimilated, the results are excellent. However, in many cases of nose sculpting surgery, cartilage grafts have a tendency to move, absorb or extrude. Use of Gore-Tex is not only viable, but a superior alternative to cartilage grafts in most cases.

Gore-Tex is a very new effective implant material. It has undergone extensive testing. It is inert and safe for patients use and has been approved by the FDA for surgical use.

Nasal Tip Reconstruction with Cartilage Graft

Dr. Chugay has found that cartilage grafts produce an excellent reconstructive result in the case of patients requiring nasal tip reconstruction. When the nose has been damaged, in previous surgeries or traumatic injuries, to such an extent that a major nasal tip correction is required, cartilage grafts are sutured into the nasal tip to reconstruct the tip. This is a rare and unique procedure which requires enormous skill on the part of the surgeon. Because of its complexity, open rhinoplasty surgery is performed to access the nasal tip.

Reconstruction of Severe Nasal Deformity

Frequently, correction of septal deformities will allow for straightening of crooked noses. Occasionally a conservative removal of most of the septum is the only effective solution to straighten out a severely misshapen nose. Such removal of septal cartilage (inner barrier between the nostrils) is a fundamental revolution in rhinoplasty technique allowing for correction of previously thought-of-impossible nasal deformities. Such nose sculpting surgery of septal removal, nevertheless, preserves the tip support. Dr. Chugay, in the United States, and Dr. Jose Juri, in Argentina, have popularized this technique.

Who should Perform Reconstructive or Secondary Rhinoplasty?

Successful Reconstructive or Secondary Rhinoplasty (nose sculpting surgery) is limited to a handful of highly skilled surgeons. Make sure that your surgeon has experience in these procedures. Only about 5% of surgeons in the United States are familiar with and use Gore-Tex implants.

After Your Surgery

Following the surgery, the nose will be placed in a cast to allow the area to heal free from external damage. The cast will be removed on your post- operative visit. There will be swelling in the area and breathing may be difficult post-operatively; however, as the swelling decreases you should notice a dramatic change in ease of breathing.

Chin Sculpting Surgery, Rego Park, New York

Some people have a problem where the chin is disproportionately small when compared to the rest of the face. This is condition is commonly referred to as micrognathia and may sometimes be referred to as “mandibular hypoplasia.” When one has a lack of chin projection, the face is weakened structurally. It can also make the nose look larger than it really is. For this reason, many patients will seek surgery to improve the appearance of the chin. The operation can also greatly enhance the results of a Facelift, since loss of chin projection may occur with aging. What is Unique About Dr. Chugay’s Chin Augmentation?

Dr. Chugay uses a custom made, narrow-shaped implant to create a more pronounced slim, yet powerful chin. The implant is inserted on top of the bony ridge and wraps around the chin bone. The placement along with the materials used cause implant shifting to be extremely unlikely.

Implant Materials Available Commercially

The most common implant material used for implants is silicone. Silicone is inferior to Gore-Tex and acrylic as material for a chin implant. Below is some comparative information on the aforementioned materials.

Silicone – Easy to mold and bend. Most silicone chin implants have a tendency to move after surgery. Unless ridges are created, silicone is very smooth and slick. Until the body creates a protective fibrous pocket, silicone implants can move and produce protrusions. Silicone, although biologically inert, causes temporary tissue reaction.

Gore-Tex – a brand name for Expanded polytetrafluoroethylene (patented and manufactured by W.L. Gore &Assoc., Flagstaff, AZ), which is a pure, microporous form of Teflon that is thin, flexible, mechanically strong and has a high degree of biocompatibility. It is easy to mold and bend. Natural soft appearance. Less movement is described after insertion when compared to silicone. No patient discomfort reported as Gore-Tex is extremely lightweight and soft. Gore-Tex is perfect for Chin Augmentation when minimal improvement is required. Please, see Cheek Augmentation.

Acrylic – Hard bone-like material. Very light in weight. No patient discomfort is noted. Once inserted, acrylic implants are hard to dislodge as they attach to the bone. No tissue reaction as with silicone. If you touch a chin augmented with acrylic you will not be able to tell the implant from the bone: a very natural blend.

Implant Shapes

Most implants are shaped to fit on the ridge of the chin bone. This, combined with the silicone’s slippery nature, often causes post-operative implant shifting ruining the surgery results. Furthermore, most implants are very wide. While appropriate for some patients, they typically appear disproportionate, especially on women and patients with smaller faces.

New Line of Implants Designed by Dr. Chugay

After years of research, Dr. Chugay has developed a line of implants, which will become a powerful addition to the implants he currently uses. The new implants come in variety of shapes and sizes to accommodate different patients’ needs. The hallmark is the downward elongation that is produced with the implant’s insertion, giving the patient a more chiseled appearance.

Of course, Dr. Chugay must see you to determine which implant shape is best for you. Each person’s face is unique. In some cases, a custom implant made just for you can be recommended. You can finally attain the chin shape you desire!

The Procedure

Dr. Chugay will make a small incision underneath your chin. If requested, Dr. Chugay can also make an incision in the mouth. However, that technique, preferred by many surgeons, will not yield as good a result as the under-the-chin placement. An exterior approach minimizes the possibility of infection and allows more precise positioning of the implant. Post-operatively, the outside scar is not visible as it is hidden underneath the chin and is only 1 cm in length. Furthermore, Dr. Chugay uses his unique technique of subcuticular suturing which reduces the scar size to the absolute minimum.

What Exactly is Augmented?

  • Hollow cheeks may be congenital, result from the natural aging process, or appear as the result of autoimmune diseases (eg AIDS) or chronic illnesses. Unfortunately currently available AIDS medications will often cause very noticeable dips in patients’ cheeks. Dr. Chugay’s procedure for submalar augmentation is a very effective way to eliminate the hollows and give the cheeks a natural full appearance.

What is unique about Dr. Chugay’s Procedure?

Implants

After years of research, Dr. Chugay has brought together the best materials and shape conformations for a natural-looking cheek augmentation.

Material

Currently Dr. Chugay uses implants made of a soft, porous material called Gore-Tex. Unlike most implants (still in use by the vast majority of surgeons) which are made of silicone, Gore-Tex implants:

  • Have a more natural appearance
  • Have not been known to move (silicone implants are usually quite unstable as they have a slick, smooth surface)
  • Incorporate better into the body tissues without causing tissue reaction
  • They usually cannot be detected by touching the area around the augmented cheekbone
  • Are very lightweight (patients do not feel the implants and do not experience the discomfort sometimes associated with silicone implants).

What is Gore-Tex?

Gore-Tex is a brand name for Expanded polytetrafluoroethylene (patented and manufactured by W.L. Gore &Assoc., Flagstaff, AZ), which is a pure, microporous form of Teflon that is thin, flexible, mechanically strong and has a high degree of biocompatibility. It is composed of Teflon fibrils held together at nodal junctions with tightly cross-linked fibrils of PTFE, resulting in greatly improved mechanical strength (up to 5300 N). It has a relatively smooth surface, minimal foreign body response and low infection rate.

Shape of implants

The implants that Dr. Chugay currently uses have an excellent natural, anatomically correct appearance and incorporate well into almost anyone’s face.

Why do some people’s cheeks look so fake after a Cheek Implant procedure?

This has to do with the type of implant used. Most cheekbone augmentations are still done with old-fashioned wide silicone implants, which often create the artificial appearance.

On some people this contour actually looks good. On many it looks artificial. Dr. Chugay’s current line of implants is designed differently, creating a more natural and aesthetically pleasing look.

The Procedure

A small incision is made inside the mouth and the implant is positioned on the bone: either above the cheekbone in the case of cheekbone augmentation or on the bones in the submalar region in the case of submalar augmentation. The implant is secured and the incision is closed.

After your Sugery

After surgery, you’ll be wrapped in an elastic bandage or a surgical bra over gauze dressings. A small tube may be placed in each breast to drain off blood and fluids for the first day or two.

You may feel some pain for the first couple of days -especially when you move around or cough-and some discomfort for a week or more. Dr. Chugay will prescribe medication to ease any pain and discomfort.

The bandages will be removed a day or two after surgery, though you’ll continue wearing the surgical bra around the clock for several weeks, until the swelling and bruising subside. Your stitches will be removed in one to three weeks.

If your breast skin is very dry following surgery, you can apply a moisturizer several times a day, but be sure to keep the suture area dry.

Even though, your results may not be as good in the long-term, you will have the puffiness in the short-term.

Is Autologous Fat Transfer Better Than Collagen Injections?

WHAT IS THE BEST AGE TO HAVE A FACELIFT?

There is no magical age to make the decision to have a facelift. The appropriate time to have a consultation is when you no longer like what the mirror tells you. This may be in your 30s or your 80s. However, it is best to start at a relatively young age, before deep lines and wrinkles set in. It is then a matter of maintenance.

HOW LONG DO FACELIFT RESULTS LAST?

There is no magic figure; however, a good estimate is 7-20 years. It is not uncommon for people to have several facelifts to maintain the young look. With Dr. Chugay’s Vertical Facelift technique, your results will last longer than with a standard procedure.

WHAT IS A WEEKEND FACELIFT? CAN I GO TO WORK THE NEXT DAY?

A weekend facelift is a term coined by the media. There is no such thing. There is, of course, microdermabrasion. However, it will NEVER give you the same results as a Vertical Facelift.

Dr. Chugay’s Vertical Facelift technique is very aggressive, yet recovery is rapid. A facelift, nonetheless, is considered to be a major operative procedure. Like any extensive surgical procedure, operative incisions and complicated surgical stitching is required. Contrary to popular belief and despite some newspaper and magazine articles, this is not as simple as having your hair set or a getting a facial.

HOW SOON WILL I SEE THE RESULTS?

AFTER SURGERY: HEALING AND RECOVERY

Immediately after the Vertical Facelift surgery, bandages are placed about the head and face for 24 hours. A scarf may be worn to cover stitches until they are removed. Keep bandages and incisions dry until sutures are removed.

In the Vertical Facelifts, patients can expect some swelling of the face and eyelids and some discoloration is common with any type of facial surgery. This is most notable on the second and third days and diminishes in approximately one week. However, this also depends on your type of skin, age and healing ability.

Make-up may be applied over the skin of the face up to the edge of the surgical scars approximately seven to 14 days following the facelift surgery. No hair coloring or stripping should be performed until Dr. Chugay gives you permission. Other hair treatment procedures may be carried out approximately two weeks following surgery.

Do not allow anyone to pull on your hair or roughly massage the scalp. A hair dryer, if used, should be set at medium to cool heat.

Do not wear earrings until sensation has returned to your earlobes.

After your Vertical Facelift surgery, social activities may be temporarily limited. Strenuous exercise should be avoided for four to six weeks.

Post-surgical questions should be directed to Dr. Chugay. It is unwise for you to accept advice from individuals unfamiliar with your specific operation.

The Principle

The crux of Dr. Chugay’s methodology is based on the premise that a curved lip that promotes slight upper teeth visibility will enhance the natural beauty of smile. This type of curvature is common (yet, not universal) for young people. As we grow older, upper lip begins to droop ruining our smile and broadcasting our age. By reshaping the upper lip curvature, Dr. Chugay is able to undo what age or genetics inflicted on your smile.

History of Upper Lip Reshaping

Open Lip Reshaping In 1923 Kilner and Gillies developed a procedure aimed at improving the contour of the lip via creating a new lip vermillion (upper border of the lip) slightly above its original position. Variations of this technique are still in use by some surgeons. Of course, the prime disadvantage of such methodology is the visible scarring, which will occur subsequent to such open lip reshaping. Gonzales-Ulloa Technique In 1971 Cardoso proposed a removal of superior portion of the lip next the base of the nose. Gonzales-Ulloa in 1975 combined this procedure with a horizontal removal of excess skin along the crest of the lip. This methodology is known to some surgeons and may be appropriate in some old patients. Its major drawback is visible scarring. Bullhorn Procedure It is unclear who pioneered this technique. It involves making a straight or V-Shaped incision at the base of the nose and removing excess skin to improve the angle of upper lip. The crucial disadvantage of this procedure is visible scarring under the base of the patient’s nose. Other Techniques Since introduction of the aforesaid techniques, very few original procedures have been described. Among those, all have proven to be merely variations of the Kilner and Gilles, Bullhorn and Gonzales-Ulloa Techniques.

Dr. Chugay’s Smile Enhancement Procedures

Dr. Chugay’s smile procedures incorporate three distinct techniques: Seagull Procedure External Lip Shortening Technique Internal Lip Shortening Technique All of the above methods involve both new and time-tested techniques and methodologies. During your consultation, Dr. Chugay will advise you if you are a candidate for one or several of these techniques.

Seagull Procedure

The incision is made inside each nostril and extended underneath the base of the nose to minimize the scarring. An elliptical removal of upper lip skin is conducted, and then tissues are closed in layers. This procedure is similar to Bullhorn Technique in that it removes the skin near the base of the nose lifting the vermillion (upper crest of the lip) and the lip in general. However, Seagull Techniques has several distinct advantages: Scars are hidden inside the nose. Tissues are closed in layers. The sutures are absorbable. Hash marks and suture marks are avoided.

External Lip Shortening Technique

This procedure is based on the methodology originally described by Gonzalez-Ulloa insofar as it involves shortening of the lip muscle via the microcoagulation technique (small electrode is used to create areas of coagulation that upon healing will shorten the lip muscle. Following are the departures from the Gonzales-Ulloa technique: This technique is usually used in conjunction with the Seagull procedure. Since the incisions are made inside the nose, the scars are hidden (in the Gonzales -Ulloa procedure, the muscle was accessed via incisions made on the outer skin of the lip). The microcoagulation is conducted only at the upper portion of the lip muscle (to avoid nerve damage and preserve lip sensitivity).

Internal Lip Shortening Technique

This revolutionary technique is, in fact, quite straightforward. Dr. Chugay makes a horizontal incision in the mucous membrane of the lip (inside the mouth) and places permanent sutures in a vertical fashion to shorten the lip muscle. Meticulous subcuticular closure of the mucous membrane is conducted. This technique represents a complete departure from all previously described lip enhancement procedures and is most effective for those with excessively long upper lip.

What causes crow’s-feet

With time, small wrinkles form, eventually growing to deep lines. In a young person Crow’s-feet are visible only during smiling; age solidifies them into permanent wrinkles.

Old Solutions to Eliminating Crow’s-Feet

Most traditional solutions to Crow’s-feet are not lasting. Since the majority of them involve tightening of the skin around the eyes, they only represent a temporary fix. The powerful orbital muscle will form new Crow’s-feet in short order.

Laser Skin Resurfacing

Although successful in making facial skin look younger, lasers are quite dangerous and unpredictable. The most effective CO2 laser has been known to cause changes in skin coloration or even scarring, especially on the tender skin around the eyes, where the laser is often applied very aggressively to remove deep lines. Erbium Yag Laser, although safer, is also less effective in rejuvenating the skin.

And yet, laser skin resurfacing is undoubtedly a revolutionary method of rejuvenating facial skin — except for one little stubborn area — Crow’s- feet, where laser therapy is largely ineffective. Laser, when applied aggressively, will help remove superficial wrinkles around the eyes; however, new lines will appear very quickly; sometimes in a matter of months.

Chemical Peels

Peels are just like laser therapy — very effective on the facial skin in general, yet useless when it comes to Crow’s-feet elimination.

Silicone Injections

Unfortunately, injecting silicone around the eye area does not yield a natural look and may even leave an unpleasant discoloration at the injection site.

Collagen Injections

The problem with collagen is that it rapidly absorbs. By the time one heals; the collagen is gone, and the wrinkles are back. Furthermore, the wrinkles are not eliminated; they are just slightly masked, just like in the case of silicone. This is not a lasting solution.

Extended Blepharoplasty

This variation of Blepharoplasty (skin tightening around the eye) involves extending the incision beyond the rim of the eye into the temple area. This allows the surgeon to remove excess skin in the temple area – where the Crow’s-feet are found. Unfortunately, this technique will not reduce orbital muscle tension. Furthermore, the extended incision will leave a visible scar in the temple area, which is entirely undesirable.

Botox

To date, Botox has been a singularly effective treatment for Crow’s-feet. Its success is due to the fact that it attacks the cause of Crow’s-feet (orbital muscle tension) by temporarily paralyzing the orbital muscle.

BOTOX has two distinct disadvantages. First, the treatment will produce only temporary improvement. For up to 6 months, BOTOX (Botulinum Toxin A — a neurotoxin) will slacken the orbital muscle. As the effects of the neurotoxin wear off, the muscle will come alive and start vigorously producing new wrinkles.

The second disadvantage stems from the fact that BOTOX is not universally effective. On some patients muscle slackening is significant. Yet, on many, it is barely noticeable.

Ineffectiveness of old Methods

Crow’s- feet, perhaps the earliest manifestations of facial aging, have proven to be quite unresponsive to current interventions. The reason for the dismal long-term results of such surgical intervention relates to the facts that efforts have been directed to the effect (wrinkles) rather than to the cause (muscular stress around the eye). BOTOX – the only cause-based treatment — is temporary and frequently ineffective.

Precursors of Dr. Chugay’s Methodology

Some surgeons have attempted to remove the orbital muscle altogether causing ugly deformities and the inability to smile. Some have tried to lengthen the muscle by incising it. However, stubbornly, the muscle would re-contract, causing even deeper Crow’s-feet.

Dr. Chugay’s Solutions

With a stroke of a genius, Dr. Chugay found a simple and yet, brilliant solution. After studying the various muscle segments which cause skin tightening around the eye, Dr. Chugay has discovered that by removing certain, carefully measured, segments of the orbicularis muscle, a permanent reduction or utter elimination of Crow’s-feet can be achieved.
